Dosing schedule for GLP-1s

Whether you are taking a GLP-1 for weight management, type 2 diabetes, or another condition, most GLP-1 drugs follow a structured step-up dosing schedule.

Here are a few dosing examples:

GLP-1S FOR WEIGHT MANAGEMENT

  • Usually start at 0.25 mg given once weekly.
  • A doctor will increase your dose every 4 weeks, starting at 0.5 mg, then 1.0 mg, and finally 1.7 mg.
  • You will eventually reach your maintenance dose.

  • The starting dose is 1.5 mg once daily for 30 days.
  • A doctor will increase your dose every 30 days, starting at 4 mg and progressing to 9 mg.
  • You will eventually reach a maintenance dose of 25 mg once daily.

  • Zepbound injections generally begin at 2.5 mg given once weekly.
  • A doctor may increase your dose by 2.5 mg every 4 weeks until reaching an effective maintenance dose.
  • The maintenance dose can range from 5 mg to 15 mg.

GLP-1S FOR TYPE 2 DIABETES

  • Typically, Ozempic injections start at 0.25 mg given once weekly.
  • A doctor will increase your dose every 4 weeks, starting at 0.5 mg and progressing to 1.0 mg.
  • If the drug has not reached its desired effects, your doctor may increase your dose to 2 mg given every 4 weeks.

  • Usually start at 2.5 mg per week for 4 weeks
  • After 4 weeks, a doctor will increase your dosage to 5 mg per week. You’ll stay at this dosage long term if it works well for you.

If you miss a GLP-1 dose

If you miss a dose of GLP-1, you may be able to take it right away or wait until your next scheduled dose. It depends on the medication you take and how many doses you’ve missed.

How to handle missed injections

This depends on when your next scheduled dose is due. Specifically, if you miss a dose and your next scheduled injection is due in:

  • more than 48 hours, take the missed dose as soon as possible
  • less than 48 hours, skip the missed dose, then wait for your next dose at the usual time

If it has been 2 or more weeks since your last GLP-1 dose, call a doctor before resuming. They may have you:

  • continue your GLP-1 treatment at the usual dose on the next scheduled day, or
  • restart GLP-1 treatment at a new dose

What to know: Never inject more than one dose of your GLP-1 at a time.

How to handle missed doses for tablets

  • If you forget to take your daily GLP-1 tablet, skip the missed dose.
  • Then, take the next pill the following day.

What to know: Never “double up” (take two pills) on any given day to make up for a missed dose.

If you think you took too much

If you inject or take more GLP-1 medication than the prescribed amount, you may develop serious side effects.

It is important that you do not use more than a doctor advises. Doing so may lead to harmful effects.

GLP-1 overdose symptoms

  • severe nausea
  • severe vomiting
  • severe low blood sugar, which may cause:
    • tremors
    • sweating
    • heart palpitations
    • dizziness
    • rapid heart rate
    • confusion
    • fainting
    • seizures

What to do in case you take too much GLP-1 medication

Call a doctor right away if you believe you have taken too much GLP-1 medication. You can also call 800-222-1222 to reach America’s Poison Centers or use its online resource.

If you have any severe symptoms, call 911 (or your local emergency number) or go to the nearest emergency room immediately.
